Thanks to support from a three year grant from California Pacific Medical Center, staff at Bayview Hunter's Point Multipurpose Senior Services can help seniors find health services in the neighborhood and at CPMC through the agency's Health Navigator.

Realizing the lack of access and the dramatic health issues affecting African Americans, the need for information and support was recognized by CPMC.

Special presentations regarding health care services are also scheduled at community and church sites. 

A faith based outreach worker also visits community churches to provide information and referral.

This grant also provides for staff training and conferences to enable the staff at Bayview Hunter's Point Multipurpose Senior Services to upgrade their skills.

Health Access Program assists a minimum of 30 seniors per year to access preventive health care services from CPMC and services in the neighborhood.

Health Access Program provides outreach to a minimum of 20 African American based churches per year, resulting in a minimum of 40 referrals of seniors per year who will access at least one of the services at CPMC or in the community.

Health Access Program improves the quality of services for seniors served by BHPMSS by providing additional training for staff.
