Bayview Hunter's Point Adult Day Health Center provides seniors and disabled adults with the services they need to prevent excessive hospitalization and nursing home placement.

The program is designed to assist participants with physical and/or mental challenges. Many seniors aand disabled persona are at risk of nursing home placement. Through the services offered at the center, participants are able to remain in their own homes. 

In addition, the AARP money management services ensures that participants' bills are paid on time, that financial problems such as credit card debt are addressed, and the financial abuse is prevented.

Hot meals are served each day and each participant has an individualized plan of care based on skilled nursing, personal care, physical-occupational-speech therapies, nutritional counseling, social services and therapeutic recreation.

Our participants are cared for by people who understand their unique health, cultural and family issues and who provide personalized care.

Staff focus on providing preventive care to help keep local residents out of hospitals and nursing homes.

Services provided help participants retain their mobility and ensure consistent access to health care - something that has been missing throughout much of their lives.

Individuals are accepted who often have very high level health issues.

Bayview Hunter's Point Adult Day Health Center is licenced by the Department of Health Services.

Wheelchair accessible transportation is available, as well as noon meals and a snack.
